Who needs a DA 1059?
This form is used if military personnel attend service school in the USA or abroad. The DA 1059 form makes it possible to control the studying process of an individual at the service school. The form is completed by the rater and reviewing officer as well.
What is the DA 1059 for?
It’s very important to keep a record and monitor the training of military personnel at various service schools as it will influence their military records. This Academic report provides a qualitative and quantitative assessment of student’s abilities. The Academic Evaluation Report is submitted to the Official Military Personnel file after the student graduates from a military school.
Is the DA 1059 accompanied by other forms?
This form should be accompanied by the degree awarding transcripts of the student.
When is the DA 1059 form due?
This document is completed before the student leaves the school.
What information should be provided in the DA 1059 form?
While completing the form, the rater is to indicate the following:
- Date of issuing
- Student’s name, SSN, Grade, Specialty
- Course title, name of school, duration of course
- Performance summary (choose the appropriate box)
- Abilities of the student (written communication, oral communication, leadership skills, contribution to group, research ability)
- Academic potential for higher level training
In case the student had problems during their study, the rater must write down some comments on the student’s academic performance).
The form should be signed by the rater and by the reviewing officer. They also have to type their names, grade, branch and title.
The rated student must sign and date the form as well.
Where do I send the DA 1059-1 form?
The original copy of the Academic Evaluation Report is given to the student. One copy is forwarded to the soldier’s Commander and to the U.S. Human Resources Command.
